Address Change Form 
The address change form should be used by current employees who need to change their home or campus address. Click on the link above to access the Address Change form. Print out the form, complete the required fields at the top of the form (including your signature), and then enter the new information for your campus or home address. Return the form to the Human Resources Department at 111 Alumni Hall.

In addition, if an employee changes permanent residence to a new municipality or has just established permanent residence in Pennsylvania, then a Local Wage (Earned Income) Tax form should be completed. This will insure deductions for the local wage tax (usually 1%) are sent to the correct municipality. Click on the link above to access the Local Wage (Earned Income) Tax form. Print out, complete, and sign the form. LIST ONLY ONE MUNICIPALITY - this is where the local tax will be sent. Return the form to the Human Resources Department at 111 Alumni Hall.
